OBSERVATION: The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. REGULATION GUIDANCE: This form must be uploaded to the California Environmental Reporting System (CERS). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility.This is a repeat violation, Class II.111CCR 2637, 2638(e), 2643(g) Failed to notify the EHD 48 hours prior to conducting testing. OBSERVATION: Annual monitoring system certification, leak detector testing, and spill container testing were performed on 10/01/2019 without providing notification to the EHD at least 48 hours in advance. Testing was last performed on 12/03/2018 and was due by 10/2018. REGULATION GUIDANCE: Notification shall be provided to the CUPA at least 48 hours in advance of testing. CORRECTIVE ACTION: The EHD will not accept these results and testing must be rescheduled with a 48-hour notification. Immediately schedule these tests and provide 48 hours notification to the EHD.This is a Class II violation.112CCR 2638, 2641(j), 2715(f) Failure to have properly qualified service technician test leak detection equipment every 12 months. OBSERVATION: Annual monitoring system certification and leak detector testing were previously performed on 12/03/2018 testing was due 10/2018. Testing is now completed on 12/16/2020, twenty six (26) months past due. REGULATION GUIDANCE: These tests are required once every 12 months. CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner.This is a repeat violation, Class II.113CCR 2637(a-d), 2712(b)(1)(F) Failure to conduct secondary containment testing. OBSERVATION: Secondary containment testing was previously performed on 4/22/2015 and was completed on 07/18/2018, three (3) months past due. REGULATION GUIDANCE: Secondary containment testing is required once every 36 months.
