The incorrect and missing information included the following from the inspection reports: <br />- The DO reports of 9/25/2020 and 8/27/2020, the OWNER / OPERATOR failed to provide a name, signature, and <br />date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES <br />section. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />This is a minor violation. <br />302 <br />CCR 2630(d), 2641(a) Leak detection equipment not installed/maintained/located to detect leaks at earliest <br />opportunity. <br />OBSERVATION: The UDC 3/4 sump sensor was raised approximately 2-3 inches off the lowest point of the sump <br />and not located to detect a leak at the earliest opportunity. <br />REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br />possible opportunity. <br />CORRECTIVE ACTION: The sensor was relocated to the lowest point of the sump by the service technician. <br />Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity. <br />This was corrected on site. <br />This is a repeat violation, Class II. <br />304 <br />CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product -tight. <br />OBSERVATION: Approximately two gallons of liquid was observed in UDC 3/4 indicating a leak in the primary <br />containment. <br />REGULATION GUIDANCE: All primary containment for the UST system must be product tight. <br />CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the <br />failed component(s), under permit and inspection of the EHD if necessary. <br />This is a Class II violation. <br />FA0003911 PR0231470 SCO01 01/25/2021 <br />EHD 23-01 Rev. 9/16/2020 Page 6 of 7 Underground Storage Tank Program OIR <br />1868 E.
