I quantity shall be reported in the facility's business plan. This violation <br /> 2 occurred January 1 , 2013 and was corrected June 5, 2019. The violation was <br /> 3 cited March 28, 2016. <br /> 4 3.6 The Respondent violated HSC § 25505(a)(2), failed to complete and/or <br /> 5 submit a site map with all required content. A site map was not submitted as <br /> 6 part of the business plan. A site map shall contain a north orientation, loading <br /> 7 areas, internal roads, adjacent streets, storm and sewer drains, access and <br /> 8 exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> 9 material handling and storage areas, and emergency response equipment. <br /> 10 This violation occurred January 1 , 2013 and was corrected June 5, 2019. <br /> 11 The violation was cited March 28, 2016. <br /> 12 3.7 The Respondent violated HSC § 25505(a)(3), failed to establish and/or <br /> 13 submit emergency response procedures for a release or threatened release. <br /> 14 The emergency response procedures were not completed and submitted to <br /> 15 CERS. This violation occurred January 1, 2013 and was corrected June 5, <br /> 16 2019. The violation was cited March 28, 2016. <br /> 17 3.8 The Respondent violated HSC § 25505(a)(4), failed to submit a <br /> 18 training program or the training program is not reasonable or adequate for the <br /> 19 facility. A training program was not submitted as part of the emergency <br /> 20 response and training plan. Training programs for all employees and annual <br /> 21 training, including refresher courses, shall include safety procedures for the <br /> 22 event of a release or threatened release of a hazardous material, including, <br /> 23 but not limited to, familiarity with the facility's emergency response plans and <br /> 24 procedures. This violation occurred January 1, 2013 and was corrected June <br /> 25 5, 2019. The violation was cited March 28, 2016. <br /> 26 4.
