The following is an itemized list of underground storage tank violations that have not been addressed for TOKAY SHELL as of September30 , 2021 . OBSERVATION Annual monitoring system certification and leak detector testing were previously performed on time on 01 /31 /2018 . It was performed late on 04/ 10/2019 and 05/07/2020 . The annual monitoring system certification and leak detector test is due in January annually. The test was performed on 04/21 /2021 , 3 months late . REGULATION GUIDANCE: These tests are required once every 12 months. OBSERVATION Annual monitoring system certification testing was performed on 05/07/2020 and a test report was submitted on 08/28/2020 , 3 months late . REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests .
