OBSERVATION: The following maintenance and monitoring records for the last three years were not found on site: -The 12/20/2019 overfill prevention equipment inspection form. REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 208 CCR 2716(a-e)Failure to comply with the designated operator inspection requirements. OBSERVATION: Designated operator reports were not signed and did not contain corrective actions for compliance issues within the required time frame. -The 12/22/21, 10/4/21, 9/7/21, 8/9/21 designated operator reports were not signed by the owner/operator. -The 10/4/21, 9/7/21, 8/9/21, 6/7/21, and 2/26/21 designated operator reports did not contain a description of each corrective action taken or to be taken on identified compliance issues. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. -------------------------------------------------------------------------------------- OBSERVATION: The designated operator failed to check and note the correct test dates for the overfill prevention equipment inspection and the monitoring system certification on the designated operator inspection reports. The correct date for the overfill prevention equipment inspection is 12/20/2019, and for the monitoring system certification is 12/21/2020. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
