A NJ O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ADELFO'S ARCO AM/PM 3443 CHEROKEE RD, STOCKTON December 28, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 305 HSC 25290.1,25290.2, 25291 Failed to maintain secondary containment(e.g.failure of secondary containment <br /> testing). <br /> OBSERVATION: Brine was found in the diesel STP and fill sumps and a brine stain was found in the 91 STP sump, <br /> indicating a leak in the secondary containment. <br /> REGULATION GUIDANCE: Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to make repairs <br /> to the UST system under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635, 2637.2,2665, 2712 Failure to comply with overfill prevention equipment requirements. <br /> OBSERVATION: The mechanical flapper used as the overfill prevention system is not currently functional on the 87 <br /> UST. <br /> REGULATION GUIDANCE:All UST systems must be equipped with an approved,functional overfill prevention <br /> device at all times during delivery operations. <br /> CORRECTIVE ACTION: Immediately discontinue deposition of petroleum into this tank until the component is <br /> repaired or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the EHD. <br /> If the failed component can't be replaced immediately,there is a possibility that the 87 UST system may be red <br /> tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> 1. OBSERVATION: UST Tank Information forms are not current in the California Environmental Reporting <br /> System (CERS). The overfill protection section is incorrectly marked "yes"for audible/visual alarms, although <br /> the overfill protection equipment inspected today was fill tube shut-off valves. <br /> REGULATION GUIDANCE: Any change of information must be updated in CERS within 30 days of the <br /> changes. <br /> CORRECTIVE ACTION: Immediately log into CERS, update the required information, and submit for review by <br /> the EHD. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 1/27/2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
