The <br /> Facility Employee Training Certificate dated 11/20/2020 is missing the facility's CERS ID and employees trained that <br /> day assumed responsibility before they were initially trained . <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a repeat violation, Class II. <br /> 304 CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Oil liquid was observed in UDCs 5/6, 11/12, and 13/14, indicating a leak in the primary <br /> containment. <br /> REGULATION GUIDANCE:All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a repeat violation, Class II. <br /> 305 HSC 25290.1,25290.2, 25291 Failed to maintain secondary containment(e.g.failure of secondary containment <br /> testing). <br /> OBSERVATION:Approximately 2 cups(each)of liquid was found in the 87 and 91 fill sumps, indicating a leak in the <br /> secondary containment. <br /> REGULATION GUIDANCE: Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. <br /> CORRECTIVE ACTION: Immediately contact a properly licensed,trained, and certified contractor to make repairs <br /> to the UST system under permit and inspection of the EHD. <br /> This is a repeat violation, Class ll. <br /> FA0023733 PR0541416 SCO01 10/13/2021 <br /> EHD 23-01 Rev.9/16/2020 Page 9 of 11 Underground Storage Tank Program OIR <br /> 1868 E.
