SAN J O A R N Environmental Health Department <br /> i , �V <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: —F <br /> Sunwest Chevron 2758 W Kettleman Ln, Lodi October 13, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br /> OBSERVATION: The Veeder-Root 304 sensor in UDC 3/4 failed to activate an audible and visual alarm when <br /> tested to ensure continuous monitoring of the tank system. <br /> REGULATION GUIDANCE:All monitoring equipment shall be maintained to activate an audible and visual alarm or <br /> stop the flow of product at the dispenser when it detects a leak. <br /> CORRECTIVE ACTION: The sensor was replaced and retested on 11/15/2021. <br /> This is a Class II violation. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 87 and 91 spill containers failed when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: The fill adapter was readjusted and the spill containers were retested during the inspection <br /> and passed. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on 11/18/2021 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 12/19/2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
