Aft <br /> W 45 Facility Number: <br /> SOLID WASTE FACILITY PERMIT 39-AA-0051 <br /> 17. Lead Enforcement Agency(LEA)Conditions: <br /> a. The operator shall comply with all State Minimum Standards for solid waste handling as specified in Title 14,California Code of <br /> Regulations(CCR). <br /> b. The operator shall maintain a log of special/unusual occurrences. This log shall include,but is not limited to,fires,explosions, <br /> the discharge and disposition of hazardous or unpermitted wastes,odor events,complaints,and significant injuries,accidents or <br /> property damage. Each log entry shall be accompanied by a summary of any actions taken by the operator to mitigate the <br /> occurrence. The log shall be available to site personnel and the LEA at all times. <br /> c. Additional information concerning the design and operation of the facility shall be furnished upon request and within the time <br /> frame specified by the LEA. <br /> d. The maximum permitted daily tonnage for this facility is 500 tons per day and shall not receive more than this amount without a <br /> revision of this permit. <br /> e. This permit is subject to review by the LEA and may be temporarily suspended or revoked at any time by the LEA for sufficient <br /> cause,in accordance with Division 30 of the Public Resources Code,Part 4,Chapter 4,Article 2, Sections 44305 et seq.and <br /> associated regulations. <br /> E The LEA reserves the right to suspend or modify waste receiving and handling operations when deemed necessary due to an <br /> emergency,a potential health hazard,or the creation of a public nuisance. <br /> g. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is <br /> prohibited. Such a change may be considered a significant change,requiring a permit revision. In no case shall the operator <br /> implement any change without first submitting a written notice of the proposed change, in the form of an RFI amendment,to the <br /> LEA at least 180 days in advance of the change. <br /> h. A copy of this permit shall be maintained at the facility. <br /> i. The LEA shall be notified in writing of any change in ownership or operator of the facility per section 44005 of the Public <br /> Resources Code(PRC). <br /> j. The facility shall meet the design and operational standards of Title 14 CCR,Sections 17850-17870. <br /> k. Physical contaminants removed from the feedstock,compost,or ground material shall be removed from the site within 7 days <br /> and transported to a disposal facility. Disposal receipts must be kept on site and available for review upon request of the LEA. <br /> 1. Stockpiling of compostable organic material for a combined period of time greater than six months is considered disposal and <br /> must be either removed from the site or processed for composting operation. <br /> m. In accordance with Title 14 CCR, Section 17867.5,operators shall ensure that all personnel assigned to the operation shall be <br /> trained in subjects pertinent to operations and maintenance,physical contaminants and hazardous materials recognition and <br /> screening,with emphasis on odor impact management emergency procedures. A record of such training shall be maintained on <br /> site. All personnel working with the load screening program shall be trained to recognize hazardous wastes,and shall be trained <br /> for safety and evacuation procedures. <br /> n. Wastes to be composted must go directly into the composting operation and may not be stockpiled on site for more than seventy- <br /> two(72)hours. <br /> o. Leachate control is required. <br /> p. Current and future stockpiles shall not exceed 25'in height, 150'in width,and 250'in length. <br /> q. Specific operational standards that this site must meet are as follows: <br /> 1. Site shall not receive more than 500 tons per day or 156,000 tons per year. <br /> 2. Site shall not receive more than 75 vehicles per day. <br /> 3. Litter must be controlled on site at all time. <br /> 4. Site shall comply with their LEA approved Odor Impact Minimization Plan(GIMP). The OIMP shall be revised to reflect <br /> changes in operation,and a copy shall be provided to the LEA within 30 days of those changes per Title 14 CCR, Section <br /> 17863.4(c). <br /> 5. Dust control shall be maintained to prevent limiting the visibility of personnel on-site and from creating a nuisance off-site. <br /> Page 4 of 5 <br />
