SAN JOAQUIN CCUNT Environmental Health Department Underground Storage Tank Program Inspection Report Facility Name: Facility Address: 1 Date: Harney Lane 76 141 E HARNEY LN, LODI August 18, 2021 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item # Remarks OBSERVATION: The designated operator incorrectly documented the test dates for the Secondary Containment on the designated operator inspection report as 7/7/2020 on the 2/15/2021 report. This is a VPH site and we have no records of a secondary containment test being conducted. The designated operator incorrectly documented the test dates for the Overfill Prevention inspection on the February 2021 - May 2021 designated operator inspection report as 7/7/2021. Per the EHD records the Overfill Prevention Inspection was conducted on 6/4/2020. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. OBSERVATION: The designated operator employee training for Taranjit Singh was not performed. Tarajit was the only employee on site during the inspection and stated that he started two weeks ago. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
