A document was submitted on 8/28/12 stating a timeline for the completion of the project. Also a <br />complaint was received by the EHD stating that the facility was operating outside of the permitted hours of <br />operation (before 7:00 am). During the 8/28/12 meeting, the operator admitted that an employee has been <br />onsite fixing equipment before the permitted hours of operation. <br />1. According to permit condition 17(x)(13), the scale must be moved to a new location to allow the queing of <br />three trucks. <br />2. Continue with and complete the relocation of the scale by the agreed upon timeline. <br />3. According to permit condition 4(b), the facility is only allowed to operate (which includes fixing vehicles <br />and equipment) within the hours of 7:00 am an 4:00 pm. <br />4. Conduct operations only within the permitted time. <br />x <br />17867.5 - Personal Training <br />Comments: There were several pieces of treated wood waste (TWW) in the woody material pile. <br />1. Operators shall ensure that all personnel assigned to the operation shall be trained in physical <br />contaminants and hazardous materials recognition and screening. <br />2. Conduct a TWW recognition and screening training. Submit documentation of the training (description of <br />the training and an attendance sheet) during the next routine monthly inspection. <br />
