On December 30, 2020, Vicky Vang-Lee of the San Joaquin County <br />EHD performed a paperwork inspection and a full inspection report was provided. <br />An amended inspection report was issued on December 31, 2020 identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted 30 days <br />after receiving the inspection report. This information has not been received resulting in a non- <br />compliant status for this facility. <br />The outstanding violations are itemized on the following pages and may include violations from past <br />inspections. Complete and submit a copy of the Return to Compliance Certification form to the EHD <br />with a statement documenting the corrective actions that have been or will be taken of each violation, <br />and any supporting paperwork. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, <br />is billed at the current hourly rate ($152).
