He stated that masks are worn by employees who <br /> want to use them. Kitchen staff has been vaccinated for COVID. He also stated that oil is changed daily. <br /> Observations: Hard to reach floors in kitchen and walk in coolers are not clean. Noticed that floors at the end of the hallway <br /> (were rest rooms are located)are stained.Walk in wire shelves are moldy and there is a foul odor in walk in with raw chicken. <br /> About a quarter of the staff wear mask. Kitchen does not look messy but hall way in kitchen and rest room hall has lots of <br /> merchandise. Oil is fryers does not look old. Only one kitchen employee has stained nails on his right hand only. He stated <br /> that it is caused from handling raw mushrooms. Hand wash station has soap and paper towels.
