Complaint Investigation Summary Report#:5105 <br /> COMPLAINT ID: COQXZNQTU Site Location: 800 E PESCADERO AVE <br /> Activity Summary <br /> Activity Date Recorded by <br /> 06/23/2020 5:48pm Met with Brandon Espinoza, Supervisor,All Environmental Services. (209) GAITAN <br /> 648-8060. Looked inside the storm drain and observed water contaminated with <br /> diesel still inside, storm drain appeared approximately half full. Brandon stated that <br /> total amount of contaminated water generated from clean-up is at 26,000 gallons. <br /> He stated that it would take longer than anticipated. Brandon stated another 3 <br /> hours will be needed for clean-up to be completed. <br /> Left site at 6:24pm <br /> 06/23/2020 8:06pm sent text to Brandon Espinoza. He stated all water has been pumped out GAITAN <br /> and they are currently working on removing the sludge. <br /> Arrived at 9:12pm Met with Brandon and Jim Carroll, Hydraulic Operator for ABC <br /> plumbing. They were still pumping out the storm drain water contaminated with <br /> diesel. <br /> Jim from ABC plumbing brought to my attention that water was entering from the <br /> West side of the storm drain. Brandon and I looked to see if drainage plug was <br /> leaking. Brandon stated that water might be seeping through. Contacted Garrett <br /> Backus(Lead Senior REHS) discussed water seeping through. After discussion I <br /> observed the flowing water in the storm drain for approximately 2 minutes, I <br /> observed no diesel sheen in the water. Determined the storm drain to be <br /> adequately cleaned. <br /> Total waste generated from clean-up so far: <br /> -Two 21,000-gallon tank filled with water contaminated with diesel. <br /> -One 21,000-gallon tank partially filled with water contaminated with diesel. <br /> -Three 55-gallon drums with absorbent contaminated with diesel. <br /> -One 20-yard bin tank approximately a quarter filled with sludge contaminated with <br /> diesel. <br /> Note: three 300-gallon totes with water contaminated with diesel were pumped out <br /> and transferred to one 21,000-gallon tank with water contaminated with diesel. <br /> All waste was left on site and will be managed pending on hazardous waste <br /> testing. Brandon stated that all waste will be managed as hazardous waste until <br /> testing results are provided. <br /> Brandon stated that he is leaving the booms and absorbent around storm drain <br /> pipe until tomorrow, as a precaution. He stated that he will contact me when he <br /> comes back out to remove the drainage plug and booms and absorbent. <br /> Sent text to Tricia Taniguchi, (415) 720-8858 updating her clean-up. <br /> Left site 11:12pm. <br /> Arrived home at 12am. <br /> 06/24/2020 Was updated of spill by Jason Gaitan. The storm drain was cleaned. The ALANIZ <br /> absorbent booms were not removed. <br /> 06/24/2020 Met with Brandon Espinoza, owner for All Environmental. Brandon removed storm GAITAN <br /> drain plug prior to my arrival. All Environmental crew removed sandbags and <br /> booms from storm drain pipe entrance and removed booms from creek. I <br /> observed no sheen in the water. I determined clean-up to be adequate. <br /> Contacted Elena Manzo (lead Senior REHS) updated her on clean-up. <br /> 06/25/2020 Discussed clean-up and site disposition with John Alaniz (REHS) GAITAN <br /> 06/25/2020 Generated photo log from photos taken on 6-23-2020 and 6-24-2020. Sent copy of GAITAN <br /> photo logs to John Alaniz (RENS). <br /> 5105.rpt Page 8 of 11 <br />
