The following is an itemized list of underground storage tank violations that have not been addressed for MIRACLE MILE MARKET as of March 09 , 2021 . Open violations from August 25 , 2020 inspection Violation #208 - Failure to conduct the designated UST operator visual inspection at least once every 30 days . OBSERVATION : The designated operator failed to check and note all required items on the visual inspection reports . The incorrect and missing information included the following from the inspection reports : -incorrect overfill test dates -Section VII was incomplete -service technician certification was expired on 02/23/20 for the 03/05/2020 report -04/09/2020 report was signed with a prior date of 01 /06/2020 -some reports were not signed by the owner/operator -missing secondary test date REGULATION GUIDANCE : During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION : Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . -- - - - - - -- - - --- ---- - - ---- - -- -- - -- - - - - - -- -- - -- ---- - - ----- ---- - OBSERVATION : The designated operator inspections for the 04/09/2020 was performed more than 30 days after the previous inspection conducted on 03/05/2020 .
