1
Florido, Elianna
From:Florido, Elianna
Sent:Friday, November 1, 2019 4:06 PM
To:'Delise, Danielle'
Cc:'Hojnicki, Jeremy'
Subject:RE: Ulta Beauty #1152 - 2019 Hazardous Waste Inspection
Attachments:Ulta 1152_ 1423 Lower Sacramento_10.7.19 Haz Waste Inspection_RTC Worksheet.pdf
Hi Danielle,

Thank you for your email with the return to compliance documentation for Ulta Beauty #1152. The following
information is needed to close out the remaining violations:

1. Violation 102 – Submit a statement explaining how the facility will manage absorbent (including powder
absorbent, used paper towels, and wipes) used to clean-up a chemical/product spill in the facility moving
forward.
2. Violation 106 – According to the retraining certification signoff sheet that you provided, only the store
managers and a coordinator were re-trained. Are these all of the employees at that store? Is there a difference
between managers and associates? Did the salon stylists receive training? Any employee (managers,
coordinators, associates, stylists, etc.) at the store that handle or manage hazardous waste should be re-
trained, and proof of this re-training should be submitted.
3. Violations 110 and 111 – Submit a copy of manifest 012494967FLE (6/20/2019) with the designated facility’s
signature, at the bottom of the manifest (items 19 and 20).
4. Fill out and submit the Return To Compliance Certification (page 1 only), attached.

Please let me know if you have any questions.

Thank you,

Elianna Florido, REHS
San Joaquin County Environmental Health Department
Registered Environmental Health Specialist
1868 East Hazelton Ave
Stockton, CA 95205
P: (209) 468-0343 | F: (209) 468-3433 | E: eflorido@sjgov.org

Please note that my email has changed to eflorido@sjgov.org
