No modified contingency plan <br />Ulta Beauty #1152 had an Emergency Response Contingency Plan at the time of inspection. The plan was accepted by <br />the CUPA in CERS on January 30, 2019. The store has printed the existing contingency plan from CERS and placed a <br />hard copy of the plan in the backroom hazardous waste area- photo attached. Managers have reviewed and acknowledge <br />that they understand the plan. <br /> <br />105. No emergency coordinator <br />The emergency coordinator at Ulta Beauty #1152 is the General Manager. The store has posted a copy of an Emergency <br />Procedures sign next to the telephone in the backroom listing the telephone number for primary emergency coordinator <br />and alternate emergency coordinators- attached photo. <br /> <br />106. Failed to train employees on hazwaste <br />Ulta Beauty #314 associates receive training on waste handling and emergency procedures during employee on-boarding <br />and throughout the year. Since the inspection, all store managers have been re-trained on waste handling and emergency <br />procedures- attached. Managers reviewed and understand Ulta’s Emergency Response Procedures binder located at the <br />front cash wrap in addition to posting emergency procedures, contact information and map by the backroom phone. <br /> <br />109. Failed to send generator manifest copies to DTSC <br />Ulta Beauty’s hazardous waste vendor Stericycle sends copies of all generator manifests to the DTSC within 30 days- <br />attached. Please note, Stericycle does not mail manifest copies to the DTSC using a delivery method that confirms <br />shipment or delivery. Stericycle has been informed by DTSC that the DTSC is substantially delayed on the uploading <br />manifest copies. <br /> <br />110. Failed to keep manifests for 3 years <br />All Ulta Beauty stores, including #1152 maintain signed copies of manifests from the designated facility for at least three <br />years. Instead of maintaining manifests in hard copy form, Ulta Beauty stores utilize an on-line portal to access <br />manifests. All manifests can be immediately accessed for a minimum of three years. Since the inspection, managers at <br />#1152 have been re-trained on how to access manifests with the on-line portal. <br /> <br />111. Failed to comply with uniform hazwaste manifest exceptions <br />Ulta Beauty #1152 did not fail to comply with the manifest exception reporting requirements. As noted above, all <br />manifests are immediately available on Stericycle’s on-line portal. No exception reporting requirements were <br />triggered. <br /> <br />405. Failed to manage incompatibles properly <br />Hazardous waste items at Ulta Beauty #1152 were re-sorted into compatible containers and all managers have been re- <br />trained to manage waste incompatible wastes in proper containers. <br /> <br />605. Failed to label containers properly
