A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) — A sterilization certificate was not available for the Envy cartridges with lot <br />#18-11. Provide EHD a sterilization certificate for the cartridges mentioned. <br />7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste' orwith the biohazard symbol <br />and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mail back system. <br />Documentation of proper disposal shall be maintained for 3 years. HSC 119314(e) — The facility does not have a mail back <br />system or a licensed waste hauler for sharps waste disposal. Provide EHD with a contract from a licensed waste hauler <br />or a mail back system. <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available far inspection. HSC 119303 —The client consent forms did not include all the information from Form B. Include <br />the following on the client consent forms: <br />- Description of what the client should expect following the procedure. <br />- Notice that inks are no FDA approved and health consequences are unknown. <br />The client records <br />were not complete. The <br />instrument lot #was <br />not <br />written or attached to the client records and the <br />description of the <br />procedure was missing. <br />Ensure all client <br />records <br />are complete. <br />19. At the completion of the procedure, the practitioner shall provide postprocedure instructions that include all elements listed in <br />Form B. HSC 119308(b) —Ryan did not have a hard copy of the postprocedure instructions at the facility. The <br />postprocedure instructions were reviewed on the facilities website. The instructions did not consist of all elements <br />listed in Form B. Include the following on the postprocedure instructions: <br />-Signs and symptoms of infection. <br />- Indications when to seek medical care. <br />31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC <br />119314 —The faucet in the restroom was broken. The hot water would not turn on. The soap container did not contain <br />soap, it was empty. Repair or replace the broken faucet in the restroom and ensure the soap container has soap. <br />Provide photographic evidence to the EHD. <br />Reinspecion onlabout: <br />A reinspection fee <br />of $152 per <br />hour may be charged. <br />EH -11/17 <br />Page 2 of 3 <br />
