A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilites are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317— The procedure room was relocated to the front <br />of the facility. New flooring and counters were installed. The following was observed and should be repaired: <br />-There were two electrical outlets without covers in the procedure area. An electrical outlet that is located <br />behind the display case directly in front of the entrance door did not have a cover either. <br />There was an open area (hole) in the wall under the counter in the procedure area. <br />Two small holes were observed on the wall behind the display case. <br />Portions of the counter in the procedure area and the counter near the entrance were not completely painted <br />and consisted of rough areas. <br />A gap was observed in the procedure area baseboard that is located between the counter and the leather <br />recliner. <br />The procedure room wall above the mirrors and next to the entrance door is rough. <br />33. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good <br />repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth, <br />durable, and nonabsorbent finish. HSC 119314—There is an absorbent curtain between the procedure room and the room <br />with the sink. Replace the curtain with material that is nonabsorbent or remove the curtain completely. Provide <br />photographic evidence to the EHD. <br />35. Operation and <br />employee <br />training <br />records <br />shall be maintained <br />for 3 years and made <br />available for inspection. A written <br />Infection Prevention <br />and Control Plan <br />(IPCP)s <br />a e maintained, <br />followed and updated. <br />HSC 119313 — The IPCP was not at <br />the site. Maintain <br />a copy of <br />the IPCP <br />at the <br />facility. <br />Unless otherwise stated, provide evidence of all corrections to the EHD by <br />May 15, 2021. <br />NOTES: <br />1.
