OBSERVATION: The Facility Employee Training Certificate dated 02/05/2021 was incomplete. The certificate did not document the date of assuming responsibility for each employee. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a repeat violation, Class II. 307 HSC 25290.1(c3), 25290.2(c3) Failure to keep water out of the secondary containment system (post-Jul 2003). OBSERVATION: Liquid was found in the 87, 91, and diesel turbine pump sumps. Liquid was also observed in the 91 and diesel fill sumps. REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage accordingly. Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion into the 87 turbine, 91 turbine, diesel turbine, 91 fill and diesel fill sumps under permit and inspection of the EHD, if needed. This is a repeat violation, Class II. 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. OBSERVATION: The 87 and 91 spill containers failed the hydrostatic test when tested. REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. CORRECTIVE ACTION: This was corrected on site. The 91 spill container passed after ICC service technician Rhome Desbiens removed the cap, cleaned it, and retested the spill container. The 87 spill container passed after Rhome tightened the adapter and retested the spill container. This is a repeat violation, Class II.
