The certificate did <br /> not document the date of assuming responsibility for each employee. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training <br /> Certificate shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a repeat violation, Class II. <br /> 307 HSC 25290.1(c3), 25290.2(c3) Failure to keep water out of the secondary containment system (post-Jul 2003). <br /> OBSERVATION: Liquid was found in the 87, 91, and diesel turbine pump sumps. Liquid was also observed in the <br /> 91 and diesel fill sumps. <br /> REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the <br /> system by precipitation, infiltration, or surface runoff. <br /> CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 <br /> hazardous waste regulations, and manage accordingly. Immediately contact a properly licensed, trained, and <br /> certified contractor to address the water intrusion into the 87 turbine, 91 turbine, diesel turbine, 91 fill and diesel fill <br /> sumps under permit and inspection of the EHD, if needed. <br /> This is a repeat violation, Class II. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 87 and 91 spill containers failed the hydrostatic test when tested. <br /> REGULATION GUIDANCE:All spill containers shall have a minimum capacity of five gallons and be capable of <br /> containing a spill or overfill until it is detected or cleaned up. <br /> CORRECTIVE ACTION: This was corrected on site. The 91 spill container passed after ICC service technician <br /> Rhome Desbiens removed the cap, cleaned it, and retested the spill container.
