A complete report was <br /> finished on April 12, 2021 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 12, 2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152). To schedule an appointment, please call (209)468-3420. <br /> Documents provided during inspection: Return to Compliance certification, flyer for free CUPA classes, sample <br /> hazardous waste labels, hazardous waste labeling guidance <br /> Waste streams found: <br /> -Used Oil <br /> -Used Antifreeze <br /> -Oily Absorbent <br /> -Metal Fines <br /> -Drained Used Oil Filters <br /> The following minor violations were corrected on site: <br /> -No modified contingency plan. A modified contingency plan was provided and filled out during the inspection. <br /> -Failed to completely label containers or portable tanks of hazardous waste. One 250 gallon clear plastic tote of <br /> used antifreeze was observed in the hazardous waste storage area without the words "Hazardous Waste", <br /> generator name & address, accumulation start date, physical state and hazardous properties. This was <br /> corrected on site. One 250 gallon clear plastic tote of used oil was observed in the hazardous waste storage <br /> area with a faded, unreadable hazardous waste label. This was corrected on site. Accumulation start dates <br /> were determined by last disposal date. <br /> -Failed to properly manage used oil and fuel filters. One black metal 55 gallon drum of drained used oil filters <br /> was observed without an accumulation start date. This was corrected on site.
