The most recent <br /> Certification of Financial Responsibility in CERS dated 9/16/2021 is not signed.The Letter from the Chief Financial <br /> Officer is missing the signature of John F. Dentoni (CFO)and the coverage amount. <br /> REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class Il. <br /> 203 CCR 2712(b)(1-2)Failure to maintain monitoring records for 36 months and maintenance records for 60 months. <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br /> -The line and Leak detector test report dated 10/28/2019 was not found on site. <br /> REGULATION GUIDANCE: These records shall be maintained on site for at least three years.
