According to EHD's records,the overfill prevention <br /> equipment test was conducted on 9/26/2018. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> ---------------------------------------------------------------------------------------------------------------------------------------- <br /> 2)OBSERVATION:The designated operator failed to check and note all required items on the visual inspection <br /> reports.The incorrect and missing information included the following from the inspection reports: <br /> -The designated operator documented visual inspections of sumps, but left the question regarding sump sensor <br /> locations blank. <br /> -The visual inspection reports from 10/8/2020-9/17/2021 were completed on the old designated operator visual <br /> inspection report. <br /> REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required <br /> testing and maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br /> all of the required information on the reports. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. <br /> OBSERVATION: The Facility Employee Training Certificates observed on site is completed on the old training <br /> certificate. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018.
