Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COU4YU8DG Site Location: 4402 W. FOREST LAKE RD. <br />Activity Summary <br />Activity Date <br />02/08/2021 <br />Recorded by <br />received call from Dave Larsen, Heavy equipment mechanic. Stated that he was GAITAN <br />sending the photo to me. Discussed photo, Dave stated that the impact area is <br />approximately 40 feet wide and leads to a sloped field. Stated that the sloped field <br />is located away from the river, he stated that if it rains it would not flow towards the <br />river. <br />Based on conversation, I contacted Alex Steele, Environmental Field Scientist. <br />Asked when the initial spill date was. Alex stated that it was on 1-29-2021. I asked <br />why it took so long to notify CAL-OES and EHD. Alex stated that no notification <br />was made due to not being able to access the road leading to the spill. It was not <br />until today, that Dave Larsen was able to go down the road. <br />02/08/2021 Received email from Joseph Geraldes, AG Inspector. Provided contact GAITAN <br />information for Mark Beck, property owner. <br />Left a voicemail on the phone number provided by Joseph. <br />02/09/2021 Received a call from Mark Beck, property owner. Discussed complaint, stated that GAITAN <br />he wanted to speak to PGE employee in charge of conducting clean up. <br />Asked Mark if I could give Alex Steele, environmental filed scientist his phone <br />number. Mark stated I could, Mark requested if I could give him Alex's phone <br />number. I notified him that I could not do that but I would contact Alex and notify <br />him that you want to speak to him. <br />Updated Elena Manzo 'Lead Senior REHSI. <br />02/09/2021 Generated Prop 65. <br />Gave to Garrett Backus Lead Senior REHSI for review. <br />Will be mailed to Alex Steele, Environmental Field Scientist and Mark Beck, <br />property owner. <br />02/09/2021 Received call from Dave Larsen, Heavy Equipment Mechanic. Discussed <br />complaint, Dave stated that transformer oil sample would be taken tomorrow. He <br />stated that he would coordinate with Mark Beck, Property Owner, on when to <br />perform clean up. He is hoping to perform clean up by next week. <br />02/09/2021 Please note: based on phone calls received by Dave Larsen, Heavy Equipment <br />Mechanic and Alex Steele, Environmental Field Specialist there is a discrepancy <br />on when incident occurred. <br />According to Alex, spill occurred on 1-29-2021, stated that late notification was <br />due to PGE Employees not being able to reach location due to muddy road <br />conditions. <br />GAITAN <br />GAITAN <br />GAITAN <br />According to Dave, spill occurred on 1-31-2021, Dave stated he responded to spill <br />and relayed the information. <br />EHD was notified on 2-8-2021. <br />Contacted Elena Manzo 'Lead Senior REHSI updated her on conflicting <br />information. <br />02/10/2021 Received call from Dave Larsen, Heavy Equipment Mechanic for PGE. Stated that GAITAN <br />he is working with the property owner, to find the best time to perform clean up.
