4 <br />Mr. Kevin Graves <br />April 28, 2006 <br />Page 2 <br /> <br />1.0 Request for Work Plan by San Joaquin County Environmental Health Department <br />In an undated letter bearing the afterthought hand stamp date of "Oct 17 2005" the San Joaquin <br />County Environmental Health Department (hereinafter EHD) summarizes past activity and <br />reporting at the former Donna Gardner Property, located at 26056/26080 North Thornton Road <br />in Thornton (hereafter the Site, or an appropriate variant). This letter included the request for a <br />work plan to be submitted by December 1, 2005. A copy of the letter is included in Attachment <br />A. This attachment also includes a copy of Figure 2 from a report by SMITH Environmental <br />Technology Corporation that shows the locations of boreholes and monitoring wells at the site <br />that are referred to in the October 17, 2005 EHD letter. <br />The first paragraphs briefly summarize site conditions including some of the borehole <br />installation results from the early 1990s, and recent groundwater monitoring results. The <br />summary of potentiometric surface conditions in the third paragraph is inconsistent with that <br />presented on the potentiometric surface maps presented in the groundwater monitoring reports <br />submitted in 2004 and 2005 by H2OGEOL and for the earlier monitoring events by others. <br />After directing that quarterly groundwater monitoring resume the inclusion of all six monitoring <br />wells and the Thornton Municipal (from the current monitoring program including only <br />monitoring wells MW-3 and MW-4) in paragraph four, the actual request for a work plan is <br />comprised of the 69 words of the fifth paragraph (the second paragraph on page 2, as follows: <br />In addition, a work plan is to be submitted to EHD by December 1, 2005. The work proposed <br />should complete the investigation of the lateral and vertical extent of the soil and groundwater <br />contamination so that the mass of contamination remaining can be calculated and evaluated. The <br />work should also include an evaluation for performing groundwater extraction from monitoring <br />wells MW3 and MW4, and address disposal options for any extracted groundwater. <br />The October 17, 2005 EHD letter references Mr. Harlan Knoll, Senior R.E.H.S. as the point of <br />contact (i.e., case worker) within EHD; references Donna Heran, REHS, Director, EHD; and is <br />signed by Margaret Lagorio, Supervising REHS Unit IV <br /> <br />2.0 The Work Plan and the EHD Rejection Letter <br />A Work Plan to Further Define the Lateral and Vertical Extent of Contamination and Access the <br />Risk to Thornton Municipal Well No. 2 was submitted dated November 29, 2005. A copy of the <br />work plan is included in Attachment B.
