All information must be recorded objectively. <br /> • Authentic. A chain-of-custody procedure should be used, E4ch person <br /> making an entry must date and sign the document, The number of documenters <br /> shall be kept to a minimum (to avoid confusion and because they may have to <br /> give testimony at hearings or in court), Nothing should be erased. if details <br /> change or revisions are needed, the person making the notation should mark a <br /> horizontal line through the old material and initial the change. <br /> The following should be included: <br /> Chronological history of the incident, including: what happened, where, when, <br /> how, etc. Facts about the incident and when they became available. <br /> Title and names of personnel; composition of emergency action teams, <br /> Actions: decisions Made by whom; orders given: to whom, by whom, and when; <br /> and actions taken: who did what, when, where and how. Types of samples and <br /> test results; air monitoring results <br /> Possible exposures of site personnel <br /> History of all injuries or illnesses during or as a result of the emergency.A critique <br /> of each emergency response action will be conducted by management as soon <br /> thereafter as possible, The documentation so obtained will be utilized in the <br /> critique. All additional information that may be relevant will also be obtained and <br /> analyzed. The critique will include all aspects of this Emergency Action Plan and <br /> revisions thereto will be made as necessary. <br /> Emergency .Action Plan Availability <br /> Employees must be advised of this plan and it will be made available for their <br /> inspection and copying. Any employee who is uncertain of his/her emergency <br /> action responsibilities or any provision of this Plan and any employee who does <br /> not know how to obtain a copy of this plan or wants further information, shall <br /> advise his immediate supervisor at once.
