Jordan <br />Bottorff stopped working for the business around April 2020, and The Business Owner/Operator Identification form <br />was not updated and submitted to CERS within 30 days of the change. Jordan was listed as the environmental <br />contact, and the secondary emergency contact for the site. This information must be current and submitted as <br />part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />VIOLATION WAS CORRECTED ONSITE. <br />This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />250 gallons of ChemTreat 1420 were found on site that have not been reported. Any material that meets or <br />exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br />inventory, and submit to the EHD for approval. <br />VIOLATION WAS CORRECTED ONSITE. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: A site map was not completed with all the required elements and submitted as part of the <br />business plan. The site map does not indicate the location for all current chemicals store onsite. The facility has <br />removed Aqueous Ammonia from the site and the chemical is still listed on the site map. The site map does not <br />does not address the location of the internal roads, adjacent streets, and storm and sewer drains. <br />REGULATION: lA site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br />and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br />handling and storage areas, and emergency response equipment.
