OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the 8/6/2021, 5/8/2021, and 3/12/2021 designated operator inspection reports and failed to check that they were responded to appropriately. The missing alarms include: - T3 Red Diesel Overfill Alarm on 8/2/2021 - L6 Diesel STP Sump Fuel Alarm on 4/14/2021 - T2 87 Overfill Alarm on 2/16/2021 REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the inspection report. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ------------------------------------------------------------------------------------ OBSERVATION: The designated operator failed to check and note the correct test date for the 12/16/2020 overfill prevention equipment testing on the 9/3/21, 8/6/21, 7/7/21, 6/7/21, 5/8/21, 4/9/21, 3/12/21, 2/12/21, and 1/13/21 designated operator inspection reports. The overfill prevention equipment test date was incorrectly noted as 11/15/2018. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. ------------------------------------------------------------------------------------ OBSERVATION: Designated operator reports were not signed within the required time frame. -The 11/29/2021 designated operator report was not signed by the owner/operator. REGULATORY GUIDANCE: Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report, the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II.
