Violation T27CCR 20921 (a) 2 — Explosive Gas Control — The facility has explosive gas at the boundary above 5% by <br />volume. The following perimeter gas wells were monitored. The following results were noted. <br />GP 9 (S) 0.0% CH4 GP 11 (S) 13.2% CH4 GP 12 (S) 1.1% CH4 <br />(I) 0.0% CH4 (I) 23.9% CH4 (1) 3.2% CH4 <br />(D) 0.0% CH4 (D) 0.0% CH4 (D) 0.0% CH4 <br />The facility operator needs to take the necessary steps to control the landfill gas at the boundary. Any changes or <br />modifications to the gas extraction system must have prior approval from the SJVAPCD and the EHD. Review the <br />gas system operations records to determine if the gas levels are due to a line blockage or gas well field adjustment is <br />required. Review the Operations and Maintenance (O&M) manual to determine if all the required maintenance was <br />conducted. Evaluate the O&M manual to determine if maintenance in adequate. <br />2. AC T27CCR 20921 (b) — Gas Monitoring and Control — The operator submitted a report titled "Title 27 Compliance <br />Review, Perimeter Probe Monitoring Network, Forward Landfill' on September 16, 2008. The initial plan was <br />reviewed by the EHD. The EHD requested more information regarding surrounding land uses and structures located <br />near the landfill. The operator responded on December 12, 2008. The EHD reviewed the additional information <br />submitted and conducted a field investigation regarding the actual spacing of Gas probe distances. The following <br />was noted: <br />• GP is 1000ft from GP25 <br />• GP24 is 101 Oft from GP25 <br />• GP 16 is 1008ft from GP 17 <br />• GP3 is 998.5ft from GP4 <br />The plan is pending final review and comment from the EHD. <br />3. AC T27CCR 20939 (b) (1) — Sub -surface Landfill Gas Control Systems — The facility has experienced 5 subsurface <br />landfill fires. Four of the fires occurred in this year. The fires are all located along the toe of the slope. The latest fire <br />occurred a long the western toe of the old Forward Inc portion of the landfill. The fire occurred along the horizontal <br />collector. The facility operator indicated that they would place soil over the area and monitor it. The operator <br />provided a copy of the facility response plan the pertained to sub -surface landfill fires. It is currently under review. It <br />appears that the landfill gas system is over pulling and creating fires that erupt below the surface of the landfill. The <br />facility shall review and evaluate the gas control system to determine what can be done to prevent further instances <br />of sub -surface landfill fires.
