There is no direct communication between ground crews and equipment operators, <br />one of whom is in charge. It takes the LEA and a ground crew approximately 5 minutes with the car horn and <br />waving clipboard to get the equipment operator attention. <br />- The operator is to provide communication capability for the employee to communicate with the equipment <br />operator and/or person in charge. <br />X <br />20650 - Grading of Fill Surfaces <br />Comments: The surface around the active face area is rough and not graded. At the time of inspection, a truck <br />driver complaints to the LEA that the surface is so uneven that the tipper area safe. According to the driver, it <br />needs more dirt around the tipper in order for the driver to operate the tipper safely. The driver has also informed <br />one of the ground crews, but nothing was done. <br />- The fill surface is to be graded. <br />- The operator is to provide training to employee on the safety issue being brought up by drivers. <br />X <br />20810 - Vector and Bird Control <br />Comments: Several burrows are on the slope of the landfill. <br />- Repair cover damaged by burrowing. <br />- Provide adequate rodent control. <br />k }fi ' <br />Insp�ctlrsrrrt Gammen i �I <br />.E <br />Note: <br />1. All areas of concern listed on June 22, 2012 inspection report have been corrected. <br />2. After the LEA informed the equipment operator, who is in charge, regarding the safety concern being brought up by a <br />truck driver, the operator closes the tipper, and the crew places more dirt under and around the tipper area. The tipper is <br />later reopened. <br />3. The cover consists of dirt, tarp and TASW. <br />4. The concentration of explosive gas in GP8, 9, and 10B is 0% by volume., <br />5. According to the record, July 15, 2012 was the first Sunday the landfill operates on Sunday. The landfill will be <br />operating every Sunday till the Ag waste season ends or till it reaches the 20 Sundays limit. <br />6. The records are up to date. <br />
