SAN J 0 A Q I I I Irl Environmental Health Department <br /> CCUNT <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PARKWOODS GAS&FOOD 1612 W HAMMER LN, STOCKTON May 18, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 214 CCR 2712 Failure to comply with UST operating permit conditions. <br /> OBSERVATION:According to Elite IV work order 4861, the diesel product overfill prevention was repaired on <br /> 12-5-2018 and a permit was not obtained from the EHD. <br /> REGULATION GUIDANCE: Emergency repairs relating to UST system monitoring and release detection may be <br /> performed by a properly licensed,trained, and certified contractor pursuant to the EHD's policy and a permit <br /> obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by <br /> the EHD prior to beginning work. <br /> CORRECTIVE ACTION: Immediately obtain a permit from the EHD for the diesel product overfill prevention. A <br /> "Failure to Obtain a Permit"penalty applies. <br /> This is a repeat violation, Class II. <br /> 304 CCR 2631(a); HSC 25290.1(c) Failure to construct, operate, and maintain primary containment as product-tight. <br /> OBSERVATION: Seepage was detected in the shear valve areas located in under dispenser containment(UDC) <br /> 1/2, 3/4 and 7/8 indicating a leak in the primary containment. <br /> REGULATION GUIDANCE:All primary containment for the UST system must be product tight. <br /> CORRECTIVE ACTION: Immediately have a properly licensed,trained, and certified contractor repair or replace the <br /> failed component(s), under permit and inspection of the EHD if necessary. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on May 24, 2021 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 23, 2021. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
