The following is an itemized list of hazardous materials violations that have not been <br /> addressed for RUN ROADLINES INC as of September 21, 2021. <br /> Open violations from June 16, 2021 inspection <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#7 -Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER) procedures were not submitted. <br /> REGULATION GUIDANCE: A business plan shall include the following emergency response procedures for a release or <br /> threatened release of hazardous materials, including, but not limited to, the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#8 -Failed to establish and electronically submit a reasonable or an adequate training program. <br /> OBSERVATION: A training program was not submitted. <br /> REGULATION GUIDANCE: Training programs for all employees and annual training, including refresher courses, shall <br /> include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened <br /> release of a hazardous material. Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program <br /> agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, <br /> property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration the position of each employee. <br /> CORRECTIVE ACTION: Immediately prepare or update the facility's training program, log into the California <br /> Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and <br /> submit to the EHD for approval. <br /> Page 3of5 <br />
