SAN)J DAIJ I N Environmental Health Department <br /> sr <br /> C 0 U NTY <br /> Greatness grows here. <br /> Time In: 2:10 pm <br /> Time Out: 3:01 om <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: EAST MAIN DRIVE IN Date: 04/05/2021 <br /> Address: 2041 E MAIN , STOCKTON 95205 <br /> Owner/Operator: Telephone: (209)464-3172 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00053611 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> ON MONDAY 03/29/21 COMPLAINANT FOUND A BROKEN NAIL(FAKE TIP ACRYLIC NAIL) INSIDE THE MEAT OF <br /> THE BURGER. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Complaint inspection. <br /> Before attempting complaint inspection, I called complainant to obtain more information in regards to the acrylic nail found <br /> (color and sizing of nail), however the complainant was unable to answer and a voice mail was provided. I visited the facility in <br /> question and spoke with the operator in regards to the complaint.The owner stated that a customer returned with a burger <br /> and inside of the burger was a red acrylic nail, in which the customer stated they wanted a refund for their meal.The owner <br /> continued by explaining that no one on site contains red acrylic nails but would have no problem issuing a full refund for the <br /> meal. On site I observed two staff preparing food with no acrylic nails on and a cashier with lime green acrylic nails and <br /> currently missing three nails. However according to the owner,the acrylic nail observed inside of the hamburger was red.
