Stantec <br />March 8, 2013 <br />Page 2 of 5 <br />Well Destruction Report <br />PRE-FIELD ACTIVITIES <br />Prior to initiating fieldwork, all appropriate permits were obtained from the San Joaquin County <br />Environmental Health Department (SJCEHD). Copies of the permits are included in Attachment B. <br />Prior to field activities, a notification was provided to the property owners and the SJCEHD. <br />Underground Service Alert (USA) was notified prior to commencing drilling activities to identify any <br />public utility alignments that may have been located adjacent to the existing well locations. Stantec <br />also contracted a geophysical survey company to identify any private utility alignments that may <br />have been in potential conflict with the drilling. The geophysical survey report is included in <br />Attachment C. Field activities were conducted under the supervision of a California Professional <br />Licensed Geologist, and in accordance with a site-specific health and safety plan (HASP). <br />WELL DESTRUCTION <br />On November 19 and 20, 2012, Stantec observed the destruction of groundwater monitoring wells <br />MW-29, MW-30, MW-32, and MW-33 located south and southwest of the Mormon Slough and <br />south of the L&M OU, Delta View Apartments, and West Webber Avenue, Stockton, California. The <br />former location of wells MW-29, MW-30, MW-32, and MW-33 are shown on Figure 2. Wells <br />MW-29, MW-30, MW-32, and MW-33 were destroyed by completely over drilling the well casing to <br />remove all well materials in accordance with SJCEHD requirements. Prior to over-drilling, the <br />geophysical survey company used ground penetrating radar (GPR) scan at each location to further <br />check for utilities. The GPR scan procedures are used to ensure no existing or new utilities are <br />located adjacent to the constructed well that could be damaged during well destruction activities. <br />The wells were over-drilled using an eight inch diameter auger bit, and well construction materials <br />and debris were removed. Following removal of all well material, the resultant borehole was <br />grouted through tremmie pipe to the surface by National Exploration, Wells, and Pumps (National), <br />under the supervision of the SJCEHD. <br />WASTE MANAGEMENT <br />The well box, casing materials, and drill cuttings were contained in properly labeled 55-gallon drums, <br />and removed from the Site by Belshire Environmental Services, Inc. (Be!shire) for disposal. The <br />waste manifests are included in Attachment D. <br />CONCLUSIONS <br />Based on direction from the RWQCB-CVR in an email dated October 20, 2012 (Attachment A), <br />Stantec properly destroyed monitoring wells MW-29, MW-30, MW-32, and MW-33. The monitoring <br />wells were destroyed per SJCEHD requirements.
