Health and Safety Plan, Subsurface Investigation <br />Potential New Court Site, Stockton, California <br /> <br />4.5 Injury Reporting <br />Requirements for reporting and documentations of injuries and accidents can be found in Earth Tech <br />Environmental Practice Standard SH&E 101, Injury, Illness and Near-Miss Reporting. <br /> <br />5.0 HAZARD ASSESSMENT <br />For this project, Earth Tech will investigate the area for soil and groundwater contamination, which will <br />include drilling and soil/groundwater sampling tasks. Performance of these tasks can expose personnel <br />to a variety of hazards due to the operational activities, physical conditions of the work locations, and the <br />potential presence of environmental contaminants (see Section 3.1.3). <br /> <br />5.1 Specification of Work Tasks <br />The following is a listing of the work tasks to be performed during work and assessment activities. A <br />task hazard analysis (THA) has been prepared for each task, which specifies the major performance <br />steps, identifies the related hazards and applicable safety procedures, and specifies any additional <br />requirements (e.g., monitoring procedures). All THAs can be found in Attachment A. <br />Geophysical Clearance <br />Subsurface Soil Sampling — Direct Push Drilling <br />Groundwater Sampling — Direct Push Drilling <br />Borehole Abandonment <br />5.1.1 Unanticipated Work Activities <br />Where work activities are identified which are not addressed in this HASP, appropriate safety <br />documentation and procedures will be implemented. Prior to initiation of work activities any <br />subcontractor organization tasked with performance of such work will submit a work procedure <br />document, which presents appropriate safety procedures applicable to the specific work activities to be <br />undertaken. Submitted safety procedures will be reviewed by the SH&EP for adequacy and compliance <br />with applicable regulatory requirements and the requirements presented in this HASP.
