Wear an appropriate air <br />purifying respirator with combination <br />organic vapor/HEPA P-100 cartridges, <br />and other PPE as needed for dust. (Use <br />a North respirator or its equivalent, <br />Foam earplugs with an NRR of 33. <br />Tyvek chemical resistant suit or its <br />equivalent), <br />Traffic hazards <br />Vehicle accident. <br />Lifting hazards. <br />Delay or improper <br />performance of work due to <br />improper equipment on site. <br />Start project with Daily Health and Safety Briefing. <br />Follow safe driving procedures. <br />Employ safe lifting procedures. <br />Make sure sub-contractors are aware of their responsibilities for labor, <br />equipment and supplies. <br />Review permit conditions. <br />Develop a traffic control plan with the client/local agencies as applicable. <br />Plan may include use of cones, barrier tape, jersey barriers, etc. Include <br />drawings in the site-specific HASP. <br />It is the responsibility of the SHSO to annotate the Site Plan with the <br />Traffic Control configuration if an Approved Traffic Control Plan is not <br />available. <br />Obtain sub-contractor <br />equipment maintenance <br />records prior to <br />commencing work. <br />Improper equipment <br />maintenance, which can <br />cause equipment failure and <br />possible personal injury. <br />Verify and review maintenance records in possession are for equipment on <br />site. <br />Verify maintenance is current. <br />Set up traffic control, if <br />necessary. See site- <br />specific HASP for detailed <br />plan. <br />Wear reflective vest for traffic; steel <br />toed and shank shoes, hardhat, safety <br />glasses with side shields, and leather <br />gloves as necessary. <br />Struck by vehicle during <br />placement. <br />Vehicle accident as a result of <br />improper traffic control <br />equipment placement. <br />Use buddy system for placing traffic control. <br />Implement traffic control plan such as setting out cones and tape defining <br />safety area. <br />Adhere to approved Traffic Control Plans when working in roadways. (See <br />site-specific HASP). <br />It is the responsibility of the SHSO to annotate the Site Plan with thi <br />Traffic Control configuration if an Approved Traffic Control Plan is not <br />available. <br />Perform concrete and/or <br />pavement cutting. <br />Wear reflective vest for traffic. Wear <br />appropriate hearing protection. Wear <br />safety glasses with side shields, (or <br />face shield), Wear appropriate <br />respirator with HEPA P-100 cartridges <br />and Tyvek suit for nuisance dust. <br />Struck by vehicle during <br />cutting. <br />Noise from cutter. <br />Eye injury from flying debris. <br />Inhalation of dust. <br />Skin irritation from dust. <br />Adhere to approved Traffic Control Plans. (See site-specific HASP). <br />Wear ALL appropriate PPE, safety glasses, hearing protection, etc. <br />Supervisor/HSC must <br />confirm all corings/cutouts <br />Possible injuries and damage <br />to property due to stepping Visually inspect each and every coring or cutout. <br />Paper copies are uncontrolled and are valid only at the time of printing.
