Addendum <br />DATED: May 15, 2002 <br />PREPARED BY: AGE <br />APPROVED: x] <br />DISAPPROVED: <br />ADDENDUM NEEDED: [ Submit by: <br />ADEQUATE AND NECESSARY ELEMENTS OF THE WORK PLAN [H&S 25299.37,subd(c)(3)]: <br />To define the vertical and lateral extent of groundwater contamination, 5 off site CPTs will be <br />completed to 150 feet bgs. <br />Four groundwater grab samples will be collected for analyses from four hydrologic units from each <br />boring. <br />Groundwater samples will be analyzed for: TPH-gas, BTEX, and fuel oxygenates including MTBE. <br />COMMENTS AND CONDITIONS: <br />Six CPT locations were proposed in the W.P. EHD approves five. The western most CPT proposed <br />on the north side of Waterloo Road should be eliminated. <br />The proposed CPT location on Chronicle Avenue needs to be moved approximately 100 feet north <br />closer to the site. <br />Vertical groundwater definition near the contaminant source (MW#2) must be completed. <br />A sensitive receptor survey with a 2000 foot radius of the site must be completed and submitted to <br />EHD. Be advised, California Water Service wells STA#15 and STA#75 maybe within 2000 feet of <br />this site. <br />All cost must be preapproved by the Cleanup Fund prior to implementation of this work. <br />Interim groundwater remediation in the area of MW#2 needs to be performed. Groundwater pump and <br />treat to remove contaminant mass and aid in controlling plume migration can be done concurrently <br />with the on site and off site investigation. <br />All required EHD permits must be procured prior to drilling. <br />Tr-Regional Board Staff Recommendations For Preliminary Evaluation and Investigation of <br />Underground Tank Sites" along with its "Appendix A" and "Appendix B" should be used as guidance <br />documents.
