Hazelton Avenue <br />Stockton, California 95205 <br />Subject: <br />Well Destruction Work Plan Addendum <br />Glenn Springs Holdings, Inc. <br />Former Occidental Chemical Corporation Facility, Lathrop, CA <br />Dear Mr. Kith: <br />This Well Destruction Work Plan Addendum (Work Plan Addendum) has been <br />prepared by ARCADIS U.S., Inc. (Arcadis) on behalf of Glenn Springs Holdings, <br />Inc. (GSH), to modify the well destruction methodology described in the August <br />10, 2017 Well Destruction Workplan (the Workplan) previously submitted to San <br />Joaquin County Environmental Health Department (SJEHD) for the former <br />Occidental Chemical Corporation Facility (OCC) manufacturing facility (Site). The <br />Site is located at 16777 South Howland Road near the southwest corner of <br />Louise Avenue and South Howland Road in Lathrop, San Joaquin County, <br />California (Figures 1 and 2). The Work Plan proposed to destroy the wells by <br />pressure grouting. Based on discussions with SJEHD, the preferred well <br />destruction method is blasting with grout. This method is described below. <br />REVISED SCOPE OF WORK <br />Arcadis will retain and supervise Gregg Drilling and Testing Inc. of Martinez, <br />California (a C-57 California State-licensed driller) to destroy six wells (EW-04, <br />EW-04A, PW08-076, PW08-155, PW08-180, and INJ-14) described in the <br />Workplan (Table 1 and Figure 3). Well blasting will be conducted by McMillan's <br />Well Service, LLC (a California OSHA licensed blaster; license #9201). The blast <br />plan for each well and copy of well service blasting license are included in <br />Attachment A. <br />Well construction information is summarized in Table 1 of the Workplan <br />(attached to this addendum for reference) and presented in boring logs in <br />Attachment B.
