It is proposed that wells PW08-076, PW08-155, <br />PW08-180, EW-04, EW-04A, and INJ-14 be destroyed prior to construction activities and wells PW09- <br />080, PW09-123, PW09-198, PW09-338 be protected during construction. In addition, the conveyance <br />piping and electrical conduit associated with EW-04, EW-04A and INJ-14 will be removed prior to <br />construction. The wells and conveyance piping are not located in an area of known or suspected soil <br />contamination. Chemicals of concern (DBCP, EDB, and sulfolane) concentrations in the water table zone <br />are presented on Figure 4. <br />SCOPE OF WORK <br />The well destruction activities consist of pre-field activities, field activities, and reporting. Site-specific <br />details are provided in the following sections. Site plans showing the locations of the wells proposed for <br />destruction are included on Figures 2 and 3. <br />Pre-Field Activities <br />Updates to the site-specific health and safety plan (HASP), permitting and underground utility clearance <br />activities will be conducted prior to well destruction, as described in the following subsections. <br />Health and Safety <br />The site-specific HASP will be updated to include procedures and hazards specific to well destruction <br />activities and associated piping removal to protect workers and the community. The HASP will be <br />approved by a designated Health and Safety Specialist. HASP procedures will be reviewed by <br />subcontractors and field staff prior to beginning field work. <br />Permits <br />Arcadis will obtain applicable well destruction permits from the San Joaquin County Environmental Health <br />Department (SJCEHD). Arcadis will notify the SJCEHD at least 48 hours prior to commencing field <br />activities, in accordance with the permit. <br />Underground Utility Clearance <br />Underground Service Alert North (USA North) will be notified of the intent to conduct subsurface activities <br />at least 48 hours prior to commencement of destruction activities and in accordance with state law. <br />A private utility locator will be utilized to further evaluate locations for potential underground utilities (e.g. <br />pipes, electrical conductors, fuel lines, and/or sewer lines) before intrusive work is performed (in addition <br />to contacting USA North). Each well location will be cleared to a depth of 5 feet below ground surface <br />(bgs) using air-knife or hand-digging methods to further evaluate for the presence of underground utilities. <br />Field Activities <br />Field activities will include: well destruction and conveyance piping and electrical conduit removal as <br />described below. <br />arcadis.com <br />Page: <br />Lathrop Well Destruction Work Plan (8.10.2017).docx <br />2/4
