't PERMIT NO.: 1017-6MW-0537 <br />PERMITTEE: Ground Zero Analysis, Inc. <br />DATE: September 26, 2017 <br />All signs shall be installed in conformance with Caltrans Standard Plans. Upon nonperformance <br />or unsatisfactory performance by the Permittee, the Department of Transportation shall perform <br />this work and all costs for said work shall be borne by the Permittee. <br />In addition to the attached General Provisions (TR-0045), the following special provisions are also <br />applicable for installation and monitoring of groundwater monitoring wells: <br />1. Unless specifically authorized elsewhere in this permit, survey work shall not be performed: <br />On holidays or an afternoon proceeding a holiday. <br />In medians. <br />C. When work requires traffic control on lanes. <br />2. Permittee shall furnish, place and maintain required signs, safety equipment, traffic control, <br />and warning devices in accordance with Caltrans Manual on Uniform Traffic Control <br />Devices 2014. <br />All personnel shall wear hard hats and class 3 vests, shirts or jackets as appropriate. <br />Unless specifically authorized in this permit, markings within the right of way shall be <br />temporary. Paint used for markings shall be water-soluble and other markers shall be removed <br />upon completion of work. <br />Any collected data requested by Caltrans shall be furnished to Caltrans without charge. <br />Vehicles necessary for work may be parked in State Highway right of way off paved shoulders. <br />Auxiliary support and employee vehicles shall remain outside the right of way. <br />Permittee shall provide for the safe passage of pedestrians. <br />THIS PERMIT AUTHORIZES WORK ON CONVENTIONAL HIGHWAYS. <br />The State of California, Department of Transportation makes no assurance or expressed warranty <br />that the plans are complete or that the planned construction fits field conditions. Should additional <br />work or modifications of the work be required in order to meet established Department Standards <br />or in order to fit field conditions, the work shall be performed by Permittee as directed by the <br />State's Inspector. <br />4 of 5
