Ground Zero Analysis, Inc. <br />HEALTH AND SAFETY PLAN <br />FACILITY BACKGROUND <br />The Site is located at 1245 W. Charter Way in Stockton, San Joaquin County, California. The <br />property is identified as San Joaquin County Assessor's Parcel Number 163-23-008, 163.230- <br />110, 163-23-034, and 163-230-120. <br />Following the removal of underground storage tanks from the site in the early 1980's numerous <br />phases of investigation and remediation have been conducted including three years of hydrogen <br />peroxide injection. <br />WORK PLAN SUMMARY <br />Task One: Destroy twenty-five wells using drill-out method per the San Joaquin County and <br />CA State Well Standards guidelines. <br />Task Two: Destroy six wells using the pressure grout method using the San Joaquin County <br />and CA State Well Standards guidelines. <br />Task Three: Following completion of the above well destruction work, all equipment and waste <br />associated with the removal of the UST clean up will be removed. The waste that <br />was drummed or stockpiled will be profiled and disposed of at a licensed landfill or <br />licensed disposal site. <br />PAGE 2
