Directions from site to nearest hospital: See hospital and clinic route map(s) and driving directions <br />located in Attachment D, at the back of this HASP. <br />Evacuation Procedure: The Site Responsible Person will establish primary and secondary evacuation <br />locations during the "Tailgate Meeting" at the beginning of each work day. The evacuation assembly areas <br />should be located up-wind in a safe area located away from the work zone. In the event of any incident <br />requiring evacuation, all work must stop and all personnel must evacuate to the designated evacuation point. <br />After an evacuation has occurred, the Site Responsible Person will conduct a head count of all personnel. <br />Description of work: Please see attached JSA's. <br />Site Description: This is an active fuel service station. <br />Site Security: Any non-assigned personnel and visitors will be escorted at all times and will be required to <br />sign the HASP acknowledging their understanding of site safety requirements. Anyone not in possession of <br />an API "Safety Key" Card will not be allowed to perform any work on site, and will be escorted at all times. <br />ALL visitors will need to meet the minimum PPE requirements in order to remain on site. An appropriate <br />exclusion zone will be established around the work area(s) to keep the public safe and directed away from <br />hazards created by site activities. Proper signage shall be posted at the exclusion zone to inform public of <br />hazards associated with the work area. <br />Stratus Environmental prescribes to a "Weapons-Free Workplace" policy. All Stratus and subcontractor <br />employees, vendors, and visitors to the site are strictly prohibited from bringing any type of weapon onto the <br />site, or possessing a weapon while engaged in business for Stratus Environmental for any purpose. <br />Personnel found to be in possession of a non-work related weapon (ex. guns, knives, explosives, chemical or <br />biological weapons) will be removed from the site and subject to disciplinary action, and possibly criminal <br />charges. <br />PPE Requirements: Protective clothing and equipment (PPE) is intended to shield or isolate personnel <br />from chemical, physical and environmental hazards. Minimum Level D PPE shall be worn at all times while <br />in operational areas and shall consist of: <br />High visibility vest meeting a minimum ANSI/ISEA 107-2004 Class 2 standard. <br />Head protection (a hard hat that meets ANSI Z89.1 standard is required) <br />Eye / face protection (shall meet the requirements of ANSI Z87.1.) consisting of appropriate safety <br />glasses with side shields, goggles or a face shield. <br />Foot protection (shall meet the requirements of ANSI Z41.1.) <br />Long sleeve outer garment. The preferred cloth is cotton or wool.
