SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Site Control and Safety Procedures <br />May 4, 2017 <br />(e.g., septic leach fields, etc.) during intrusive activities, intrusive activities include, but not limited <br />to drilling, soil boring, well installation or abandonment, excavating, trenching, air knifing, <br />auguring, grading, driving rods or piles, potholing, or installing posts. <br />During the pre-job planning phase, the Stantec project team is responsible for: <br />Reviewing all available site plans (e.g. client-provided "as-built" drawings or previous <br />Stantec site plans) for potential site features (i.e., utilities, underground infrastructure, <br />etc.) and sign off on the Stantec Pre-Ground Disturbance Worksheet & Approval Form <br />and CEMC Ground Disturbance Review Checklist. Forms are located in Attachment 2 of <br />the HASP. <br />Completed forms must be signed by the Stantec Project Manager, approved by the CEMC <br />Project Manager, and attached to this HASP for workers' reference on-site. <br />When on-site and before intrusive work begins, the SHSO is responsible for: <br />Reviewing the completed SWP 213a Pre-Ground Disturbance Worksheet and CEMC <br />Ground Disturbance Review Checklist forms, <br />Completing Stantec's SWP 213b Ground Disturbance Form, and (if applicable) CEMC's <br />Well Abandonment Review Checklist, <br />Locating all overhead hazards such as elevated utility lines, utility poles, or canopies, <br />If any work locations or circumstances constitute high risk activity (in accordance with <br />the Permit to Work Process), then CEMC Project Manager's acceptance on the PTW is <br />required before work begins. <br />Work Hours <br />Work on this project will be conducted during the hours of 7:00 AM to 7:00 PM, Monday through <br />Friday. <br />43
