SITE-SPECIFIC HEALTH AND SAFETY PLAN FOR WELL INSTALLATIONS <br />Site Control and Safety Procedures <br />May 4, 2017 <br />portion of the property parking lot. The exclusion zone will be set around individual drilling <br />locations using barricades and other delineators. <br />Stop Work Authority <br />All on-site personnel are empowered, are expected, and have the responsibility to stop their <br />own work and the work of co-workers, EMC employees, or other contractors if any person's <br />safety or the environment is at risk. NO repercussions will result from this action. <br />Site or project conditions that are possible reasons to stop work and to consider modifications to <br />the HASP include: <br />Recognition of new or unidentified hazards <br />PPE breakthrough or unexpected degradation <br />Unusual odors that can't be identified <br />Unexplained, elevated readings on an organic vapor monitor <br />Unexpected changes in soil coloration or texture that might indicate undisclosed <br />contamination. <br />This list is not comprehensive and should be used only as guidance. Whenever Stop Work <br />Authority is exercised, the occurrence should be documented. <br />Traffic Guidance and Control (review and update to be site specific). <br />Incidents on sites have shown the need for a site-specific Traffic Guidance and Control Plan. <br />The SHSO and project staff will develop a Traffic Guidance and Control Plan and disseminate <br />this information to all site personnel. This plan will consider the amount of traffic at a site and <br />provide for the safety of all workers. Equipment and resources to be considered as part of traffic <br />guidance and control include: <br />Vehicle hazard lights (tail and headlights) <br />Delineators and orange construction fencing <br />Placement of vehicles as barriers between workers and traffic <br />Rotating amber hazard lights that can be placed on top of vehicles <br />Professionally trained flaggers <br />Other considerations for the Traffic Guidance and Control Plan include: <br />Requiring personal vehicles (that aren't being used as barriers) to park as far away from <br />potential traffic as possible. <br />Cordoning off as much space as is necessary to ensure our safety. This will be discussed <br />with property owners and tenants, as it may mean closing down additional gasoline <br />pumps, etc. <br />Identifying traffic flow routes and parking areas for heavy equipment (e.g., vacuum <br />trucks, drill rigs, etc.) and establishing site speed limits. <br />Reviewing local regulations for: formally developed traffic guidance and control plans <br />signed by licensed individuals, police details, flagmen, hours of activity, closure of streets, <br />etc. <br />Utility Location & Clearance (include copies of relevant forms neededt <br />The objective of this safe work practice is to provide standard practices and procedures to help <br />mitigate the potential of encountering, puncturing, compromising or disrupting service to buried <br />and overhead utility service lines, municipal or third party owned utility services, underground <br />storage tank (UST) system components and other subsurface property service lines or systems <br />42
