S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNTY <br /> RCRA Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KAISER PERMANENTE 7373 WEST LN, STOCKTON June 22, 2021 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 118 CCR 66265.16 Failed to maintain complete hazardous waste training records. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented)that <br /> employees who handle hazardous waste were properly trained. Several containers throughout the facility were <br /> observed incorrectly labeled and open. Please see#403, 601, 604, 605. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees. Provide proof of training within 30 days to the San <br /> Joaquin County Environmental Health Department(EHD)for employees whose responsibilities include hazardous <br /> waste. <br /> This is a repeat violation, Class II. <br /> 303 CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> OBSERVATION:A spill pallet for the used oil drum was observed in the engineering hazardous waste storage area <br /> with a dark liquid in the bottom. <br /> REGULATION GUIDANCE:All facility spill control equipment shall be maintained as necessary to assure its proper <br /> operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the spill pallet in to proper working order by removing the liquid in the secondary <br /> containment and placing in the proper waste container.
