Current, accurate financial responsibility documents have not been submitted to the EHD. The dates on the financial responsibility documents have been whited out and rewritten; no new signatures were obtained. The assets and liabilities amounts on the Letter from the Chief Financial Officer have not changed in the last four annual submittals. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The map does not include leak detectors. A site plan must be submitted identifying the locations where monitoring will be performed. This is a repeat violation, Class II. 110 CCR 2637(e), 2637(f) Failed to submit "Secondary Containment Testing Report Form" within 30 days after the test. Secondary containment testing was performed on 7/11/18 and a test report was submitted on 1/2/19, five months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 113 CCR 2638(c), 2638(d) Failure to submit "Monitoring System Certification Form" within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 2/22/18 and a test report was submitted on 4/16/18, one month late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
