This information was required to be submitted within 30 days. This information was received <br />late. <br />-Inspection on 5/10/16, due 6/9/16, received 5/3/18 (22 months late) <br />-Inspection on 2/27/17, due 3/29/17, received 5/3/18 (14 months late) <br />-Inspection on 2/22/18, due 3/24/18, received 5/3/18 (2 months late) <br />An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br />statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br />written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br />inspection report. <br />This is a repeat violation, Class II. <br />201 <br />CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br />Monitoring system certification test results from 2/22/18 and secondary containment test results from 7/11/18 were <br />not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) <br />date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results <br />of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of <br />gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings <br />and reconciliations.
