Complaint Investigation Summary Report #. 5105 <br />COMPLAINT ID: CODUZ3YFW Site Location: 1-5 southbound, south of Kasson Rd <br />Activity Summary <br />Activity Date Recorded by <br />05/13/2018 12:42 pm - 4:18 pm. 3.6 hours. Received call from Program Coordinator BACKUS <br />Muniappa Naidu, reporting this accident and spill. I called CHP dispatch at <br />943-8600 who described the spill as a truck accident, fire and 20 gallon diesel spill <br />on the dirt shoulder. CHP requested EHD response. <br />I arrived on site at 1:30 pm and discussed the incident with CHP Sergeant <br />Rickman and Cal-trans Haz Mat specialist Juan Armendarez. The truck caught <br />fire and spilled fuel on the roadway and down the outer southbound shoulder. <br />I called the responsible party contact Josh Carter with CTI Leasing and asked him <br />who is conducting the shoulder cleanup and their estimated time of arrival. Josh <br />said the company Environmental Logistics should be calling Juan of Caltrans. I <br />told Josh my department will be overseeing the cleanup and will be sending an <br />invoice for the EHD time spent on this incident. He provided the company mailing <br />address in Indiana. Josh also provided the company email to receive the Notice to <br />Abate. <br />I gave Juan the Environmental Logistics contact and he called them to arrange the <br />cleanup. Juan spoke with Marcial Barragan, 1-510-670-9901, <br />mbarragan@environmentallogistics.org and said that USA will survey the spill area <br />for utilities. The cleanup will occur some time next week. <br />The California Department of Transportation (Cal-Trans) conducted the roadway <br />cleanup using a street sweeper and will keep the roadway absorbent waste at the <br />local Cal-Trans yard until pickup by Environmental Logistics. <br />All Star Heavy Haul removed the fire-damaged truck and pieces of the truck. Aron <br />of All Star also said they removed automotive fluids from the truck and placed <br />them into two poly drums. Aron was planning on hauling the drums with the truck. <br />I asked him to leave the drums on site at the spill location unless he is hauling the <br />drums as hazardous waste with a manifest. After discussion with CHP and <br />Caltrans, Aron agreed to leave the drums on site, down the embankment, on top <br />of the spill site. <br />I wrote and emailed the notice to abate, photos and the spill report to Josh Carter <br />at dispatch@ctitruck.com by 4:18 pm. <br />In the notice to abate I told the responsible party to contact the San Joaquin <br />County Environmental Health Department (EHD) at 209-468-3420, when the spill <br />cleanup is scheduled, to witness the contaminated soil excavation. I told the <br />responsible party to provide the disposal records for the contaminated soil, <br />roadway cleanup waste and water-fuel mixture to the EHD to complete the <br />cleanup. <br />05/15/2018 Emailed dispatch@ctitruck.com and asked when the cleanup will be done. I <br /> BACKUS <br />received email back from T Riherd Safety Director of CTI Truck stating the matter <br />has been referred to Protective Insurance. <br />05/17/2018 Juan Armendarez of CalTrans emailed T Riherd that the insurance company will BACKUS <br />not approve the cleanup work without approval from CTI Truck. <br />05/18/2018 Mailed Waste Discharge / Spill Response Notice to Abate dated 05/13/18 by MORENO <br />Certified mail to: T.
