Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: CODUZ3YFW Site Location: 1-5 southbound, south of Kasson Rd <br />Activity Summary <br />Activity Date Recorded by <br />05/18/2018 Since I will be on ER this weekend, I was informed by Garrett Backus that the FLORIDO <br />clean-up for this complaint may need to be verified over the weekend. Reviewed <br />complaint information provided by Garrett. <br />05/18/2018 Notification of Hazardous Waste Discharge BACKUS <br />05/18/2018 Processed incident photos. Emailed photos, notice to abate, notification of <br />hazardous waste discharge to T Riherd. Also emailed the relevant code sections <br />and the department hourly rate information to T Riherd. Later, T Riherd emailed <br />and called me that CURA Environmental Services will be contacting Juan of Cal <br />Trans. I told T Riherd to have CURA contact me in addition to Juan. <br />BACKUS <br />05/25/2018 Called and spoke with Jason Morgan with Patriot Environmental. Jason stated that RUVALCABA <br />they had just arrived and he would contact me when they were almost done to <br />observe cleanup before backfill occurred. <br />Jason called to inform me that cleanup was just about completed. Drove to site <br />and observed that excavation had occurred by an area located by fence. Soil was <br />moist at the time, most likely from irrigation on adjacent field. Based on <br />observations, remaining contamination could not be identified by Caltrans, Patriot <br />or myself. One twenty yard bulk container was used for the soil and vegetation that <br />was removed and two 50 to 55 gallon drums with what said to be liquid waste <br />were on site. It was stated that two other drums of waste generated on the day of <br />the accident were going to be added to the solid waste container. The solid waste <br />and the two liquid waste containers were to be stored in Caltrans property <br />approximately 2 miles north of the site. Storage of the containers was temporary <br />and would be removed after a determination of the waste had been made. <br />05/29/2018 Received green certified mail receipt in the mail. Date of delivery was 05/25/18 MORENO <br />and was signed by UNKNOWN. Certified mail #7015-0920-0001-7997-5891. <br />Gave green card to Inspector GB. <br />06/21/2019 Added photos from cleanup to file. Discovered file has been assigned to me. RUVALCABA <br />Called John Martin with Patriot Envrionmental to get manifest. Should be emailed. <br />5105 rpt <br /> <br />Page 3 of 3
