Financial responsibility documents have not been submitted to the EHD. The most recent financial responsibility documents in CERS and on site are dated 1/19/18. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class ll. 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted within 30 days. A new designated operator, Paul Godwin, performed inspections on 2/28/18, March 2018, and December 2018, and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation, Class ll. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate dated 1/9/19 contained no CERS ID, no zip code, and the "initial training dates" listed for the employees were after the "date of assuming responsibility as a facility employee". All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
